Recertification Application – Sample 1
Recertification applications are to be sent to: [email protected].
NOTE: If you wish to complete the application in MS Word [giving you greater flexibility than using the Fill feature in Adobe], please follow these steps:
- Save the Recertification Application PDF to your computer
- Open MS Word
- Use the Open command in MS Word to open the PDF.
- You may use the “Save As” feature in MS Word to save your document as a PDF once you have completed it, if you wish. The format in MS Word changes from computer to computer and doesn’t always keep your content in the form you saved it; this is due to the fact that the document saves based on your printer.
